For those Medicare beneficiaries who choose to keep their owned receiver and/or insulin pump, will suppliers use the HCPCS Level II code for supplies that existed prior to the new bundled HCPCS codes?

Effective January 1, 2028, or the date the DMEPOS CBP contract performance period for class II CGMs and insulin pumps begins, the HCPCS Level II codes A4224, A4225, A4238, and A4239 for supplies for insulin pumps and CGMs will no longer be used to bill for supplies used in conjunction with rented insulin pumps and CGMs as payment for these supplies will be bundled into the monthly payment amounts for insulin pumps and CGMs under HCPCS Level II codes E0784, E2102, and E2103. HCPCS Level II codes A4224, A4225, A4238, and A4239 would only be used to bill for supplies for insulin pumps and CGMs owned by the beneficiary prior to January 1, 2028, or the date the DMEPOS CBP contract performance period for class II CGMs and insulin pumps begins. Once all beneficiaries who owned insulin pumps and CGMs prior to the start of Round 2028 transition to rented equipment in the future, HCPCS Level II codes A4224, A4225, A4238, and A4239 will no longer be used for Medicare purposes.
